Treatment of porphyria cutanea tarda with chloroquine

Porphyria Cutanea Tarda (PCT) is a rare disorder characterized by painful, blistering skin lesions that develop on sun-exposed skin (photosensitivity). Affected skin is fragile and may peel or blister after minor trauma. PCT is essentially an acquired disease, but some individuals have a genetic (autosomal dominant) deficiency of UROD that contributes to development of PCT.

PCT patients with hemochromatosis are reportedly resistant to chloroquine, 41 and are more appropriately treated by phlebotomy, considering other adverse effects of iron overload. Specific contraindications for phlebotomy included hemoglobin 10 g/dL or hematocrit of 33, and poor venous access. Two cases of infantile porphyria cutanea tarda successful treatment with oral S-adenosyl-L-methionine and low-dose oral chloroquine. Br J Dermatol. 1987 Mar. 1163407-15. Bruce AJ, Ahmed I. Childhood-onset porphyria cutanea tarda successful therapy with low-dose hydroxychloroquine Plaquenil. Seven patients with porphyria cutanea tarda received a total of ten courses of low-dose oral chloroquine therapy 125 mg chloroquine phosphate twice weekly. Patients were treated for a mean 14.9 months during which time all went into clinical and biochemical remission.

Most individuals with the inherited enzyme deficiency remain latent and never have symptoms. These individuals are referred to as having "familial PCT".

Two cases of infantile porphyria cutanea tarda successful treatment with oral S-adenosyl-L-methionine and low-dose oral chloroquine. Porphyria cutanea tarda PCT is a term encompassing a group of acquired and familial disorders in which activity of the heme synthetic enzyme uroporphyrinogen decarboxylase UROD is deficient. Approximately 80% of all cases of porphyria cutanea tarda are acquired; 20% are familial, although the ratio may vary among different geographic regi. The frequency of porphyria cutanea tarda PCT has been increasing greatly and its incidence at present in our country can be estimated to one case in 2000–5000 inhabitants. A striking fact is an apparent shift in the incidence of the disease to younger age groups.
